General inquiries
Our website may collect your first name, email address, required phone number, five-digit ZIP code, reply preference, optional email-marketing choice, optional SMS consent, consent timestamps, submission time, generic form source and follow-up status. Your ZIP code helps us check mobile concierge availability; we do not request a street address through these forms.
How we use this information
We use general inquiry information to respond, check concierge service-area availability, administer the welcome offer when applicable and track follow-up. Inquiries are stored privately for owner review and remain there until the owner deletes them. You may contact us to request deletion.
SMS privacy
Permission to receive text messages is optional, unchecked by default and separate from providing a required phone number. We only text a website lead when that person has expressly opted in. Mobile information will not be sold or shared with third parties or affiliates for marketing or promotional purposes. Text-message opt-in data and consent will not be shared with third parties, except service providers that help deliver requested communications when properly configured.
Quiz and calculator
Find My Treatment answers remain temporarily in the open page by default. A visitor may separately choose to share a limited quiz summary with the clinic for a personalized follow-up. That optional summary is limited to the broad goal, suggested starting point, preferred next step and timing, and the consent choice and timestamp are recorded. Detailed answers and calculator selections are not included in lead records, URLs or analytics. These tools are educational and do not provide a diagnosis, prescription, dose or determination of medical eligibility.
Health-data boundary
This is a public marketing website, not medical intake. Do not submit medical histories, symptoms, medications, diagnoses, patient photographs, documents or payment-card information. Clinical intake belongs in an appropriately configured clinical system.
Services and external links
General inquiries are designed to be stored privately through ChatGPT Sites when live collection is enabled. Vagaro handles external booking. Instagram links open Instagram. This website does not install advertising pixels or analytics tracking in the approved review version.
